1. Submit the Transcript Request Form / Permission to Release Records Form to the Guidance Secretary.
2. Complete the application form using the following information:
   • CEEB Code for W.A.H.S. is:  391710
   • School address is:  2552 Route 6, Hawley, PA  18428
   • Distribute Secondary School Report and/or Counselor Recommendation Form to Guidance Secretary
   • Distribute Teacher Recommendation Forms to teachers
   • Mail application and application fee directly to the college
3. For On-line Application
   • Download the Secondary School Report and/or Counselor Recommendation Form and Teacher Recommendation Forms
   • Distribute forms to teachers and Guidance Secretary
4. Allow three weeks for teachers and counselors to complete the requested recommendations and forms.
5. Have test scores sent directly from ETS (for SAT) and/or ACT if required by the school.
6. Call the college's admissions office directly for questions regarding the status of your application.
